To ensure your vision remains at its peak, follow these simple steps to activate your SMILE® Pro Flapless LASIK guarantee at Ciputra SMG Eye Clinic Bali:

1. Procedure: Undergo a SMILE® Pro Flapless LASIK procedure at Ciputra SMG Eye Clinic Bali.

2. Eligibility Letter: Obtain an official “Lifetime Protection Eligibility Statement” from your eye surgeon at Ciputra SMG Eye Clinic Bali (issued after the third month of your post-operative period).

3. Complete Post-Op Care: Follow the mandatory post-operative check-up schedule: 1 day, 1 week, 1 month, and 3 months after your procedure.

4. Annual Eye Exams: Perform routine annual eye examinations at any Ciputra Healthcare Clinic or Hospital. If the examination is conducted outside of Ciputra SMG Eye Clinic Bali, you must provide a copy of the examination results to us.

5. Maintain Records: We highly recommend conducting your post-LASIK check-ups at our clinic with your original surgeon, as they hold your complete medical history. However, if you are unable to visit us, you may visit another eye clinic and submit the records to Ciputra SMG Eye Clinic Bali.

6. Claiming: To claim your Lifetime Protection, you must present all copies of your post-LASIK annual eye examination results.

Lifetime Protection Terms and Conditions:

At Ciputra SMG Eye Clinic Bali, patient safety is our top priority. Please review the following requirements:

• Medical Evaluation: Surgeons at Ciputra SMG Eye Clinic Bali will only perform enhancement procedures if the patient is medically cleared. Our doctors will re-evaluate your vision to ensure you are a suitable candidate for additional treatment.

• Coverage: This protection applies to vision changes directly related to the initial LASIK procedure at Ciputra SMG Eye Clinic Bali, such as Myopia (nearsightedness) or Astigmatism (cylinder).

• Exclusions: Conditions unrelated to the LASIK procedure—such as cataracts, glaucoma, macular degeneration, or diabetes—are not covered and do not qualify for enhancement.

• External Factors: Vision decline caused by other diseases, accidents, or temporary/permanent conditions unrelated to the LASIK surgery are not eligible for the enhancement procedure.

• Minimum Threshold: Enhancement procedures cannot be performed for patients with a refractive error of less than -1.00D.

• Other Restrictions: Lifetime Protection does not apply to monovision patients, individuals under 18 years old, or patients with specific residual refractive errors.

• Exclusivity: This benefit is only available at Ciputra SMG Eye Clinic Bali for patients who have undergone their initial vision correction surgery at Ciputra SMG Eye Clinic Bali.
